Where to get a DOT physical
Any medical examiner on the FMCSA National Registry can do the exam. These are the places we recommend. Ryan Contracting reimburses the exam up to $120 — keep your receipt and turn it in within 60 days.

FMCSA National Registry searchWhat to bring
- Your driver's license
- A list of ALL your medications with doses and your doctors' names
- Glasses, contacts or hearing aids if you use them
- If diabetic: recent A1C results and blood-sugar log
- If you use a CPAP: the machine's compliance report (90+ days)
- Letters from any specialist (cardiologist, neurologist, etc.)
- Page one of the DOT exam form filled out, if the clinic sent it ahead
If your examiner gives you a card that runs less than two years, that's your new expiration date — just enter what's printed on the card when you submit.
